Pride Health is excited to offer a fantastic opportunity for a Nurse Practitioner to join our client in the Minneapolis area! This 4 to 7-month contracted position not only offers a chance to make a meaningful impact but also opens doors for extensions or even a permanent role.
Join the Senior Community Care (SCC) program, a dynamic and integrated care delivery model designed to provide coordinated care for high-risk members in a facility setting. *Required to travel between 5 facilities within Minneapolis, Stillwater, and Columbia Heights*
Responsibilities:
- Conduct comprehensive assessments
- Effectively manage acute and chronic medical and behavioral conditions
- Ensure accurate and complete documentation, including ICD 10 conditions
- Communicate and collaborate with the interdisciplinary care team
- Conduct advanced illness and advanced care planning conversations
- Provide patients and caregivers with counseling and education
Qualifications:
- Active and unrestricted MN NP license
- Current, active DEA licensure/prescriptive authority.
- Ability to complete the job's physical requirements, including lifting a 30-pound bag in and out of the car, navigating stairs and various dwelling conditions, and sitting, standing, and kneeling to perform a physical assessment.
- Fully vaccinated against COVID-19
- 1 year of Long-Term Care experience preferred, but not required.
- Understanding of Geriatrics and Chronic Illness.
- Experience in meeting the medical needs of patients with complex behavioral, social, and/or functional needs.
